Our Dental Assistance Savings Plan
The Arkansas Family Dental Discount Plan is designed to provide affordability and greater access to quality dental care.
With your Dental Discount plan there are:
- No yearly maximums
- No deductibles
- No claim forms
- No pre-authorizations
- No pre-existing condition limitations
- Immediate eligibility (no waiting periods)
- Free Consultations
Program Exclusions and Limitations
- The program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan
- For services for injuries covered under workers compensation
- For treatment, which in sole opinion of the treating dentist, lies outside the realm of their capability
- For referrals to specialists, or any treatment outside Arkansas Family Dental
- For hospitalization or hospital charges of any kind
- For cost of dental care which is covered under medical
This plan is only honored at Arkansas Family Dental.
This Dental Savings Plan is not an insurance plan that can be used with any other dental office.
Program Guidelines
- There will be a reinstatement fee if your plan lapses
- Cannot be used in conjunction with another dental plan
- NON-REFUNDABLE
- No refunds or premiums will be issued at any time participant decides not to utilize the dental plan
Patient’s portion of the bill is due at the time of service.
All plans auto-renew unless Arkansas Family Dental is notified 30 days in advance.
You will not receive a membership card – your plan’s effective date will be on file with our office.
